Medical Consent
What you are agreeing to when you use Ember Clinic.
1. What you are consenting to
When you complete a screening questionnaire and book a consultation through Ember Clinic, you consent to the following:
- Ember Clinic collecting your personal and health information through the screening questionnaire
- Your screening responses and medical history being shared with the AHPRA-registered doctor who will conduct your consultation
- A phone consultation with a doctor through our clinical partner, Doccy
- The doctor making independent clinical decisions about your care, including whether to prescribe medication
2. How your health information is used
Your health information is used for the following purposes only:
- Screening: To assess whether you may be eligible for a consultation in your selected treatment category
- Doctor preparation: Your screening responses are shared with the consulting doctor before your appointment so they can review your history and make the consultation productive
- Follow-up care: To support ongoing consultations, rebooking, and continuity of care
- Record keeping: As required by law. Ember retains intake data. Doccy retains clinical consultation records.
Your health information will not be used for marketing purposes and will not be shared with any third party other than those listed below.
3. Who sees your information
Your personal and health information is shared with the following parties as necessary to provide the service:
- Your consulting doctor (via Doccy): An AHPRA-registered medical practitioner who reviews your screening information and conducts your consultation
- Pharmacy: If your doctor prescribes medication, your name and prescription details are shared with the dispensing pharmacy
- Pathology provider: If your doctor orders blood work (e.g., for testosterone assessment), a pathology referral containing your details is sent to a pathology provider
Ember Clinic staff may access your intake data for the purposes of managing your booking and providing support. All access is restricted to authorised personnel.
4. What a consultation does not guarantee
Completing a screening and booking a consultation does not guarantee that you will receive a prescription. All clinical decisions are made independently by your consulting doctor based on your individual assessment.
If your doctor determines that treatment is not appropriate, they will discuss alternatives with you. This is a feature of responsible medical care, not a limitation of the service.
5. Accuracy of information
You agree to provide accurate and complete information in your screening questionnaire. The consulting doctor relies on this information to make safe clinical decisions. Providing false or incomplete health information could affect your safety and the quality of care you receive.
6. Your right to withdraw consent
You may withdraw your consent at any time by contacting us at privacy@emberclinic.com.au. Withdrawing consent means:
- We will stop using your health information for future consultations
- Any upcoming consultations will be cancelled
- We may retain certain records as required by law (see our privacy policy for retention periods)
- Clinical records held by Doccy are retained under Doccy's own legal obligations as the health service provider
Withdrawing consent does not affect the lawfulness of any processing that occurred before your withdrawal.
7. On the intake form
Before submitting your screening and booking a consultation, you will be asked to confirm the following:
I consent to Ember Clinic collecting my health information for the purpose of arranging a medical consultation. My information will be shared with the consulting doctor to facilitate my appointment. I have read and agree to the Privacy Policy.
I understand that a consultation does not guarantee a prescription. The treating doctor will make all clinical decisions.
